Terms and Conditions
The materials and concepts of The Institute for Leadership Fitness™ are designed to increase your level of professional effectiveness and leadership fitness™. As a participant in the Program, you agree to the following terms and conditions.
Fee Schedule
The Institute for Leadership Fitness™ is a twelve-month program, consisting of four one-day sessions, held quarterly, paid annually. The fee for the annual program is $3,750, and a non-refundable deposit of $500 is required with your Enrollment Form, payable by VISA, MasterCard or AMEX (check or money order, please inquire).
Conditions Of My Enrollment
I agree to attend the four (4) sessions for which I am enrolling, which will take place within a one-year period that begins as of the date of the first session noted on my Enrollment Form. There is no credit for missed sessions.
Helping To Protect Our Intellectual Property
I agree that any commercial use of the concepts, strategies, methods, materials and all other trademarks, copyrights and other intellectual property owned by David Chinsky & Associates, LLC in any media, now known or hereafter invented, is prohibited without the express written permission of David Chinsky & Associates.
Protecting Your Personal Information
I understand that David Chinsky & Associates does not rent, sell or otherwise disclose personal information about its clients to third parties, and will use its best efforts to protect my personal information against loss and unauthorized use; therefore, I release David Chinsky & Associates and its directors, officers, agents and employees from any liability due to disclosure or use of this information.
Some Of The Rights Reserved By David Chinsky & Associates
I agree that David Chinsky & Associates has the right to reschedule workshops and/or substitute facilitators.
Responsibility For My Decisions
I understand the Program is not a substitute for the advice of any of my personal or professional advisors; therefore, I agree that all of my decisions and actions, and their consequences, are entirely my own responsibility.

Please choose the series (location) that works best for you. Complete the Enrollment Form and include the applicable non-refundable deposit - payable by VISA, MasterCard or AMEX (check or money order, please inquire). Fax to 866-693-8284.
The balance is due three weeks prior to the first session. There will be no refunds after the start of the first session of the series for which you are registered. There are also no refunds for missed sessions. A complete information package for your first session will be forwarded to you after your Enrollment Form has been received and accepted.
The Institute for Leadership Fitness™ is presently offered in the following locations: Ann Arbor, Michigan; Detroit, Michigan; Grand Rapids, Michigan; and Phoenix, Arizona. For dates and times in other locations, please call 866-960-LEAD.
Each session begins at 8:30 a.m. and ends at 5:00 p.m.
